And it's overwhelming when you're learning and you realize, oh my goodness, there's so much to check. You try and remember the oral medicine lectures. They tell you to check every single lymph node under the sun. Jaz's Introduction:And then of course, you remember the occlusion, the very few occlusion lectures you had, and you get the patient to bite together and you don't know what you're doing. You don't know what you're checking for, but you're just checking that the bite exists. And then to try and compute all the things you're seeing and try and put it in some sort of diagnosis and explanation, when you're a student, it's tricky. Which is why I've got Emma, our Protrusive Student on today to talk about routine, just routine, gold standard, daily examinations, something that you will see in general practice in the future. Whilst I am recording with Emma Hutchinson, the student, this is suitable for anyone who wants to learn, any dentist at any stage of career, who just wants to get back to basics, back to foundations. For a, what I call a Real World Examination. The reason I call it a real world examination is because the things that I'll be discussing today and what I'll be sharing video form on the Protrusive app under the student scholarship section is a real world exam. So, for example, a comprehensive exam evaluation could take an hour, 90 minutes. You ask different people. Some people do two hour full examinations. And they have my highest respect, and that's amazing. But that level of service is not for the masses. It's not for the general public. Look, some of my colleagues just get 10 minutes to do an entire examination. So I've had to find somewhat of a halfway house. I've just kind of assessed, okay, well, these are essential and these are desirable. Actually, these guidelines already exist. In the practice at the moment, we're doing these audits. When the patient feels pain during an ID block. When a matrix does not seal the cavity perfectly. When the shade of your crown doesn’t blend as well as you would like. When the endo you did last year failed. All of these cause us anxiety. All of these are failures in some form, and as Dentists, it eats at us. Let’s admit it: we are perfectionists. Life isn’t about being perfect, but it is all to do with the effort we apply to better ourselves day to day. Failure is a major part of success, but being hung up on our shortcomings will only lead to a downward spiral, which in turn will affect our results and general practice. Jaz's Introduction:And for me, I've talked about before, like for me, rubber dam is my tranquil environment. It's when I'm least stressed. I've got rubber dam on, the patient can't talk, the tongue is abolished, and I'm just falling in love with a little micro steps that makes our profession so much fun, right? But when you're isolating the last tooth, like let's say it's a wisdom tooth or whichever is the last tooth. And then that tooth also needs a restoration. Then we have some issues, i.e If you clamp that tooth, then how can you get a matrix on that tooth? And that's the problem that we're solving today with the real superstar of dentistry. And I'll give you six great reasons in reverse order. So like my top six, if you like. So six, five, four, the countdown all the way to the number one reason. Jaz’s Introduction:And when you grind your teeth left and right, the canines, for example, are taking a load at that point. And when this is happening the patient's joints are in centric relation. So the condyles are as fully seated position is snug position, and there are no quote unquote interferences or slides. I don't worry if you don't know what those things mean just yet, but some of you may remember seeing this. Maybe you don't understand or internalize what this actually means, but you learn what the perfect occlusion was. But then you started to see patients and you realize very quickly that these naturally dentate patients, your normal patients with the normal teeth. Whether they were 30, 50, or 70, they had all sorts of restorations, or sometimes no restorations. But they had anterior open bites, cross bites, and none of it looked like the bite in the textbook.

And the second one, like I said to you earlier, is I often give my patients a mirror and then get them to kind of point out what their concerns are. But actually there's so many parallels we can draw between communicating with our patients and consenting our patients for facial aesthetics and general restorative dentistry, especially like aesthetic dentistry, edge bonding, veneers. There were so many parallels. So even if like me, you have zero interest in facial aesthetics, think of this like a communication and consent episode, because this March 2024 is documentation month.
