I just signed the lease on my 500 sq ft clinic space in Long Beach, and I'm thinking a lot about what kind of practice I want to build. Instead of locking myself into a rigid model, I’m keeping things flexible—giving myself the option to shift between a cash-based walk-in clinic, urgent care, or even direct primary care. Why flexibility matters when starting a medical practice: You can adjust based on patient demand You’re not stuck with a model that doesn’t work for you You can scale up or down depending on how much patient care you want to provide I chose a retail space because I want to be visible and accessible. But if I don’t get enough traction, I’ll consider moving into a medical office building. Even in a small space, you can do a lot—bring in an NP or PA, offer virtual visits, host group coaching, and adapt your practice over time.
