Welcome back to TheCase.Report, it's our first episode of season 5! It's hard to believe that we're moving into the 5th year of TCR! All the way back in our first ever episode we had an infective case that was very topical at the time - covid - And so to kick things off for Season 5, we're looking at another highly infective case of viral haemorrhagic fever. Joining us on our case this month are our NCHDs Conor O’Gara, Leah Flanagan and Ralph Hurley O’Dwyer, who treat us to a comprehensive run through of the case. We were delighted to be joined by Dr Deirdre Morley and Dr Catherine Uhomoibhi for the second segment in the episode. They are working as co-leads on the development of the National High Level Isolation Unit (HLIU), and discuss the process of designing the HLIU as well as some Irish-led international simulation they've been leading in the Infective Diseases sphere. Season 5 is just around the corner! We'll be kicking off the new season October 7th. Mark your calendars! In the meantime, Mohammed is off to Pakistan this week to PakTraumaCon 2024. Being the insufferable nerd that he is, he's not satisfied annoying his colleagues and new acquaintances only at the conference, but has also been after them for chats before even getting on the plane... Someone needs to stop him! Joining him on this bonus is Prof Junaid Abdul Razzak, also a speaker at PakTraumaCon this year. Prof Abdul Razzak is Vice Chair of Research at Weill Cornell Medicine in New York, and is also the Director of the Centre of Excellence for Trauma and Emergencies at the Aga Khan University in Karachi. This time is always busy for our trainees with interview preparation so Leah took the opportunity to sit down with Dr Andrew Neill, consultant in Emergency and Intensive Care Medicine in the Mater Hospital to discuss the controversial topic of REBOA. They discuss the UK-REBOA trial and the evidence behind the technique and why we should be cautious about it's used in the Irish trauma system in particular. Ever wondered how to pursue an EM fellowship after finishing your SpR training?  Is it worth it, and if so what are the benefits of doing a fellowship?  We’re very excited to welcome back a familiar voice to our bonus episode this week to answer these questions and more!  TCR alumnus Dr Deirdre Breslin joined Geriatric Emergency Medicine (GEM) lead Dr Conor O’Gara to chat all things fellowships, frailty and finding out what to do with your interest in GEM.  Dr Breslin is an EM consultant in the Mater Misericordiae University Hospital (MMUH).   She has a special interest in GEM, having worked as the TCR production lead in GEM and completed a GEM fellowship prior to taking up her consultant post. With over 3900 neck of femur fractures (NOFs) reported in Ireland in 2021, these fractures represent a significant burden of morbidity and mortality, particularly for our older patients. Early recognition coupled with meticulous management, both in the ED and beyond, is instrumental in ensuring optimal outcomes.
