When Medication Isn’t Enough: Trauma, Brain Chemistry, and Character Disorders with James Morgan (Bonus Episode 47)
What is the difference between a mental health condition and an entrenched pattern of relating to other people? Can medication help someone regulate their symptoms without changing the behaviors that continue to harm those around them?In this episode, I’m joined by James Morgan of Anchored Psychiatric to explore the historical distinction between Axis I clinical disorders and Axis II personality disorders, often referred to more informally as character disorders. Although the DSM no longer uses this multiaxial system, the distinction can still help us understand why some conditions respond to medication while deeply rooted personality patterns require insight, motivation, accountability, and sustained psychological work.We discuss how medication may reduce anxiety, depression, mood instability, impulsivity, or other symptoms enough for someone to participate more meaningfully in therapy. But medication cannot manufacture empathy, honesty, accountability, or a genuine desire to change. When a person remains convinced that everyone else is the problem, treatment has profound limitations.James also explains what can happen when a child develops in an environment filled with chronic criticism, humiliation, emotional neglect, rejection, unpredictability, or fear. Behaviors that once helped a child survive may eventually become rigid and destructive patterns that affect adult relationships.We also look at the biological impact of chronic emotional and psychological abuse. Prolonged exposure to threat can shape the brain, nervous system, stress response, and the way a survivor detects danger.
