Their executives were not enforcing a culture of safety and covered up potentially risky work at their factories.  This conviction was after two earlier crashes that killed hundreds of people,  a high-profile international embarrassment that left two astronauts stranded in space and dozens of safety incidents. However, some Boeing employees claim they tried to warn senior managers about safety problems.
